Birth and Death Certificates (Vital Records)

The Montgomery County Department of Heath and Human Services issues certified death certificates when the death occurred in Montgomery County after January 1, 2015. Permits for exhumations (discontentment) are issued with the assistance of a licensed funeral home.  Birth certificates are available for any individual born in the State of Maryland.  To obtain a birth certificate at the Dennis Avenue Health Center in 1939 or after.  Those born before 1939 should contact the Maryland Department of Heath and Mental Hygiene's Division of Vital Records at 410-764-3063.

How to Apply

Bring completed application or voided the following information:  decedent's name, date and place of death, funeral home name, relationship to the deceased and applicant's full name.  A valid government issued identification (such as driver's license or U.S.. passport) is required.  The identification must have an issue date and an expiration date.

Death certificates can also be obtained by mailing a copy of the death certificate application and a copy of required identification, along with fees, to the Division of Vital Records, Reisterstown Plaza, 6764-B Reisterstown Road, Baltimore, MD  21215.

The following individuals may apply for a birth certificate:  

  • An individual requesting his/her own birth certificate.
  • A parent named on the certificate.
  • A court-appointed guardian of the individual named on the certificate.  A foster care appointment is not acceptable; proof of guardianship must be submitted.
  • A surviving spouse of the individual named on the certificate.
  • A representative of (1) the individual or a parent named on the certificate; or (2) the court-appointed guardian of the individual named on the certificate.  The representative must have a letter signed by the individual, parent or guardian; and certified by a notary public stating that the representative has permission to obtain a copy of the certificate.

Documents To Bring

For a death record complete the Maryland application for Death or provide the following information:

  • Decedent’s name
  • Date and place of death
  • Funeral home name
  • Reasons certificate is needed
  • Your relationship to the deceased
  • Applicant’s full name

You must bring valid government issued identification such as a driver’s license or passport.

Eligibility Requirements

Decedent (death certificate) must have died in the State of Maryland.  Individual must have been born (birth certificate) in the State of Maryland.

$18 for the first death certification and $20 for each additional copy.  Cash, check, credit card (Visa/Master Card) and money order.

1.  Where else can I get a death certificate? 

You may also send your application for a death certificate to the State of Maryland Office of Vital Records by fax, phone, and walk-in.  Call 410-764-3036 for information.

2.  Do you issue birth certificates in Montgomery County? 

No.  You can obtain a birth certificate by contacting the Maryland State Division of Vital Records, or call the office at 410-764-3036.
