Infant at Risk Home Visiting Program

The program accepts referrals to case manage mothers and newborns, with medical or social problems that could affect their health, development or well being. Most referrals are received from hospitals where babies are born, through the MARYLAND POSTPARTUM INFANT AND MATERNAL REFERRAL FORM through the Maryland Department of Health and targets the following conditions and circumstances:

· Teen Mothers
·  No prenatal care
·  Substance Use/Misuse
·  Mental/behavioral health
·  Intimate Partner Violence
·  Unstable housing/homelessness
·  Previous infant death
·  Previous preterm birth
·  Very low birthweight (<1500grams)
·  Any other circumstance deemed to be a serious risk for the mother or infant
