Annual Recertifications

    All personnel are responsible to download the 2012 Individual Tracking Form and have it signed off as they  complete each course. Once the entire form is completed please return the form to your assigned station officer. Your assigned station officer should verify that you have a current EMT-B, EMT-I or EMT-P state certification/license. Your station officer must verify the following information:

  • Correct Name appears on the card
  • Correct mailing address
  • Currently valid, and should document your expiration on the form
  • Verify Affiliation:  If the card has an affiliation # on it, it must read  15 0000 for DFRS, but newer issued cards no longer show an affiliation code on it.  Below is a link to download a current roster from MIEMSS for all employees that have a 15 0000 affiliation code for BLS and ALS.  Any individual that is not listed in the MIEMSS Affiliation list should be download the Affiliation/Verification Form from the link below. The individual must fill in there name and social security number (SSN) near the top of the page, and sign and date in box 4 at the very bottom of the page (an example is highlighted below). Once completed the form must be forwarded to Captain Silverman at the PSTA by e-mail or by fax (301-279-8219).

How do I prove to my Shift Officer that I passed each of the quizzes: 

When you are done taking each quiz, with the grade still showing on your screen, hold down the "windows" (see below) button on your keyboard and press the "Print Screen" button. This will capture an image of the screen you are viewing and copy it to the clipboard. Open up a word processor program such as MS Word and hit "Ctrl-V" or use the menu bar, hit the "edit" button and scroll down to "paste".  You can then print the new document to verify that you completed and passed the quiz.  70% for each quiz is passing. The AHA CPR Exam has a passing score of 85%.

CPR/AED

All stations were issued the 2010 AHA BLS for Professionals DVD last year (2011). All CPR instructors for CPR Montgomery have access to the Healthcare Provider written examinations versions C & D.

For your annual recerts, any provider that wants an AHA Healthcare Provider card must take the skills and written examinations. The skills exam must include:

1 and 2 Rescuer Adult CPR with AED

1 and 2 Rescuer Infant CPR with AED

                                      FBAO for an adult and infant both conscious and unconscious. 

Providers must pay the instructor $2.50 for each card ($5.00 for duplicate) at the time of the request. The instructor must then submit the payment and the proper course roster to the Training Center Coordinator (TCC).  The course roster MUST indicate the written score for each provider requesting an AHA card. The TCC will then send the card to the provider at their home station. NO CARD will be issued unless payment and a course roster are received by the TCC. Incomplete documents will not be accepted.
