Goal

Bullet

Improve health outcomes for pregnant women, newborns and children.
   
Target Population and Eligibility Criteria

Bullet

Children 0-2 years old who are Medicaid and Maryland Children's Health Insurance Program (MCHIP) eligible.

Bullet

Families: Medicaid eligible or have Medical Assistance; 200% of Federal poverty level (MCHIP eligible), uninsured pregnant women at 185% of federal poverty level. Infant

Bullet

Maryland Children's Health Program Premium is for children/families with incomes above the MCHIP limit, serving children up to age 19 whose family income is 201% to 300% of the federal poverty level.
 
Services Provided
Bullet
Prenatal and Child Health education.
Bullet
Care coordination which may include home visits/telephone interventions to link and coordinate with community resources (including medical provider care access).
Bullet
One hour home visit every 3 months.
