Goal

Early identification of infants with significant medical or psychosocial risks, care coordination and provision of support services as needed. Baby
   
Target Population and Eligibility Criteria

Bullet

Infants under one year old with one or more risk factors.
 
Services Provided
Bullet

Health education and care coordination for high risk pregnancies and their babies, one hour visit each week or as needed, typically for 6 to 12 months.

Access Information
Nurse

Infants identified as being at risk are referred by nurses, social workers and physicians from local hospitals or other health departments regardless of insurance or Medical Assistance status. Risk factors may include but not limited to congenital birth defects, prematurity, failure to thrive, positive drug screens, inadequate social supports.
