Triage Tuesdays
Use the triage tag as your PCR for every call on Tuesdays, beginning April 6, 2004

User's Guide

Patient Information area:
Record the bar-code number on you station run report.
Click color below to reveal color criteria.

RED YELLOW GREEN BLACK
Initial evaluation: Initial Triage

In treatment area(MCI), ambulance(non-MCI)

In ambo (MCI) or any update or change
Hospital: For the hospital to use

Circle the applicable words.
Indicate areas of complaint on the body pictures.

"X" = fracture
"/" = Laceration

Transport Unit number, Receiving hospital & time

Top 2 are for Treatment Area & Hospital
Others may be used to identify patient belongings or stuck to the seat or other area where the person was in a vehicle, etc..

Transportation record needs to be completed by the Transportation or Disposition Officer at an MCI.
On Triage Tag Tuesdays, the ambulance should complete this area & leave it attached to the tag.

 

 

Space to record 3 complete sets of vital signs.

Put additional info about past medical Hx, medications & allergies on the bottom of the tag

 

Treatment Record to record what was done, when it was done and by whom.

 

 

 

Use this area to record additional treatment, history, medications, allergies, or other pertinent information that would not fit in other areas.
