This unit accepts, processes, and issues facility and personal licenses; receives public complaints, regarding licensed facilities and rats; and distributes general information to the public.

When filing a complaint, please have the following information available:

• Name and phone number of Complainant;
• Name and location of facility;
• Date and time of the incident; and
• Description of Complaint.
