Section I – All Facilities Have you selected a proposed site for your food service operation?

YES proceed to the next question.
NO select a site then proceed to the next step.
Is your proposed site within the incorporated limits of Barnesville, Gaithersburg, Laytonsville, Poolesville, Rockville, or Washington Grove?
YES have all necessary permits regarding building, electrical, zoning and occupancy been obtained from that jurisdiction.
NO contact the Department of Permitting Services  to apply for the necessary permits and obtain zoning approval. (NOTE: Possession of a building permit does NOT indicate approval by the Department of Health and Human Services.)
Is your proposed facility on a private water supply (well) or a private waste disposal system (septic system)?
YES call 240-777-6300 for information on approval of such systems for commercial food facilities.
Do you have clearance from Washington Suburban Sanitary Commission for water and sewer?
NO contact W.S.S.C. at 301-206-8000.
Do you intend to apply for an Alcoholic Beverage License?
YES contact the Board of License Commissioners, 16650 Crabbs Branch Way, 2nd Floor, Rockville, MD 20855-2297 for requirements on submitting floor plans. Telephone number is 240-777-1999.
Note: "In Montgomery County, no license for the sale of alcoholic beverages shall be issued for any place of business located in the following election district or towns of said county: Damascus (12th district), and the towns of Barnesville, Kensington, and Washington Grove."
Do you have someone qualified to be a Montgomery County Certified Food Manager?
NO you must contact Montgomery College at 301-251-7182 to enroll in the class. The course number is FM 105
Have you made arrangements for obtaining a Trader’s License?
NO you must contact the Clerk of the Circuit Court at 240-777-9460.
Do you intend to have a charbroiler, pit barbecue, smoker, or similar cooking equipment? Is the grill area five (5) square feet or more?
YES to both you must obtain an air quality Permit-to-Construct (COMAR 26.11.18.06) from the Maryland Department of Environment before installation and operation of the cooking equipment. Do you have Workman’s Compensation Insurance or a Certificate of Compliance from the Workman’s Compensation Board?
YES provide this information on your Food Service Facility License application.
NO a copy of your trader’s license can be temporarily submitted if you are applying for a Certificate of Compliance. (Applications for Certificate of Compliance are available in our office.)
