Birth and Death Certificates

(Vital Records)

The Montgomery County Department of Health and Human Services issues certified death certificates for 30 days from the date of death for individuals who died in Maryland.  Permits for exhumations (disinternment) are issued with the assistance of a licensed funeral home. Birth certificates are NOT issued in Montgomery County.  Birth certificates for individuals born in the State of Maryland are issued through the Maryland State Division of Vital Records or by calling 410-764-3036.  

Walk-in hours to apply for death certificates are Monday through Friday from 8:30 to 11:00 a.m. at the Dennis Avenue Health Center

Bring completed application or provide the following information:  decedent's name, date and place of death, funeral home name, relationship to the deceased and applicant's full name.  A valid government issued identification (such as driver's license or passport) is required.

Death certificates can also be obtained by mailing a copy of the death certificate application and copies of required identification, along with fees, to the Division of Vital Records, 6550 Reistertown Road, Baltimore, MD  21215.
