What is Resource Coordination?
Resource Coordination assist individuals with developmental disability who are in the DDA medical assistance waiver in obtaining the best quality and most appropriate services and supports with available resources within the community. Resource Coordinators provide case management and advocate for the individual receiving services.
How does an individual become eligible for Resource Coordination?
In order for an individual to receive Resource Coordination they must be a recipient of Developmental Disability Administration (DDA) funds and eligible for the medical assistance waiver. To be eligible for DDA the individual must:
- Have a developmental disability diagnosis prior to age 22,
- Have evidence that the disability is likely to continue indefinitely, and
- Have the need for supports and assistance in activities of daily living.
Once an individual is eligible by DDA and receiving funding for ongoing services, DDA notifies Community Support Network of the individuals who may qualify for Resource Coordination services.
General Case Management referrals are sent to Community Support Network from DDA for individuals who they specify need temporary case management services.
How does an individual apply for services with the Developmental Disabilities Administration
