Transitioning Youth Services (TYS) assists students with developmental disabilities and their families in the process of graduating from the school system to enter the adult service system. This is a collaborative effort between the Montgomery County Department of Health and Human Services, Montgomery County Public Schools, the Maryland Developmental Disabilities Administration, and the Division of Rehabilitation Services. This primary goal of the collaboration is to successfully link students to adult services upon completion of the school year when the student turns 21 years of age.
Who is Eligible for Transitioning Youth Services?
Any student with a developmental disability who is a resident of Montgomery County and is enrolled in Montgomery County Public Schools or is educationally placed by MCPS in a non-public school is eligible for services.
Transitioning youth services staff work with students and their families by attending the student's Individual Education Plan meeting one to two years prior to the student's graduation from the school system at 21 years of age. Following the meeting, Transitioning youth services staff assist the student and his/her family in applying for services and mapping out a plan that incorporates the students needs and choices. Transitioning youth services staff assist students and their families in locating the best quality and most appropriate services with the available resources in the community.
