Montgomery County Department of Health & Human Services - Home Care Services - Aging and Disability Services

Provides assessment for personal care services and funding, and help with personal care to frail elderly and people with physical and/or mental disabilities who need assistance in caring for themselves. Home health aides help eligible individuals maintain an appropriate level of self-sufficiency. Call if you need the service or assessment to determine your qualifications for funding.

Administer the In-Home Aide Services (IHAS) program for people who do not receive third-party reimbursement (Medicare, Medicaid, private insurance). You must agree to pay any fee required on a sliding fee scale. Eligible applicants may be placed on a wait list for this program. In addition, emergency temporary back up coverage for personal care services is available for residents who use the Medical Assistance Personal Care Program.

Financial reimbursement to individuals with long-term or severe physical disabilities who require attendant services such as in-home assistance with personal care, household chores, and transportation. To be eligible YOU MUST:

  • Be a Maryland resident, AND
  • Be between the ages of 18 - 64, AND
  • Be determined and certified by your physician to have a severe disability that keeps you from performing essential activities of daily living, self-care, and mobility, AND
  • Not be receiving duplicative attendant care services on the SAME day (i.e. you could receive service from one program Monday to Friday and the other on the weekend), AND
  • You must be employed, OR you must be actively seeking employment, OR you must be enrolled in an institution of post secondary or higher education, OR you must be a nursing facility resident who would be able to reside in the community if attendant care is provided, OR you must be at risk of nursing facility placement if you do not receive attendant care services in the community.

Consumer is reimbursed a percentage of attendant care costs based on annual income, family size, and program funding. It is recommended to have the State's Medical Assistance Personal Care (MAPC) services or the County's In-Home Aide Services (IHAS) in place prior to applying to the State's Attendant Care Program.

Living at Home Waiver Program - Coordinating Center for Home and Community Care

The Maryland Department of Health and Mental Hygiene administers the Maryland Community Choice or Living at Home Waiver that provides services for those eligible individuals that meet a nursing home level of care and are between the ages of 18 and 64. This waiver is designed to provide consumer-directed personal assistance services for adults with physical disabilities in their own home. Waiver services include attendant care, administrative case management, family and/or consumer training, skilled nursing supervision, personal emergency response systems, environmental accessibility adaptations, and occupational and speech/language therapies. Persons receiving Medicaid and living for at least 30 days in a long term care facility are automatically eligible for the Living at Home Waiver. If the person receives both Medicaid and Medicare the wait is at least 60 days before being able to apply for waiver services.

The Medicaid Personal Care Program provides assistance with activities of daily living to Medicaid recipients who have a chronic illness, medical condition or disability. Services are provided by self-employed or agency employed providers, who are approved and monitored by a nurse case monitor from a local health department. Services are provided in the eligible individual's home or community residence.

Medicaid Waiver for Older Adults

A program to enable older adults to remain in a community setting even though their age or disability might warrant placement in a long-term care facility (nursing home). MAW allows services that are typically covered by Medicaid in a long term care facility to be provided to eligible persons in their own homes or in assisted living facilities. Services provided may include personal care, respite care, home delivered meals, assisted living services, family or consumer training, personal emergency response systems, dietitian/nutritionist services, assistive devices, environmental adaptations and assessments and behavior consultation services. Case management is provided by local AAA.

Maryland Medicaid program that is part of Maryland's managed care system known as HealthChoice. The program is intended to provide specialized services to people with special health care needs who are also eligible for Maryland Medical Assistance. To be eligible, the person must have a diagnosis that is included in the program's criteria. The individual's special needs must be considered to be outside the scope of services provided through Medicaid managed care organizations.

Provide personal care for people who are 65 years or older, have a disability and are at risk of entering a nursing home, whose income is not more than 60% or the State's median income per month, and have assets that meet the eligibility requirements.
