Primary care services for uninsured, under-insured, and disabled persons, homeless residents, and
medically needy persons encountering barriers to health care. Services include acute and chronic care (including HIV, gynecology, and preventive services); well-care for newborns, children and adults; routine immunizations/vaccines for children program; family planning health care and counseling; Diabetes education and nutrition classes in English and Spanish; preventive dental care; behavioral health counseling; referrals to medical specialists and to programs to assist uninsured patients; referrals for discount lab and radiology testing; flu shot program for patients and county shelter residents; toll-free appointment phone lines; 24-hour phone help for urgent patient concerns; and assistance with Medicaid-eligibility applications.

Free, independent non-sectarian, not-for-profit, community based primary health care provider serving uninsured, low-income adult residents. Also offer lab tests, medications, x-rays and imaging studies, and cancer screening tests.

Provide primary medical care to the low-income, uninsured, working poor, and homeless in Montgomery County. 23 health clinics located throughout Montgomery County operating at regularly scheduled times in churches, government buildings, public housing facilities, medical office buildings and on three-fully equipped mobile medical vans. Days and times of clinic operations vary. Clinics offer both appointment and walk-in service. Payment for treatment is determined by a sliding fee scale based on the patient's family income, size and ability to pay.

Provide yearly breast and cervical cancer screening and follow-up for low income, uninsured/underinsured female residents of Montgomery County aged 40 and older. Limited nursing case management services, as well as assistance linking to other community and state resources as appropriate, are available. Screening services are provided at various contracted physician and radiology facility sites throughout the County. Referral required.
