medically needy persons encountering barriers to health care. Services
include acute and chronic care (including HIV, gynecology, and preventive
services); well-care for newborns, children and adults; routine
immunizations/vaccines for children program; family planning health
care and counseling; Diabetes education and nutrition classes in
English and Spanish; preventive dental care; behavioral health counseling;
referrals to medical specialists and to programs to assist uninsured
patients; referrals for discount lab and radiology testing; flu
shot program for patients and county shelter residents; toll-free
appointment phone lines; 24-hour phone help for urgent patient concerns;
and assistance with Medicaid-eligibility applications. |

and homeless in Montgomery County. 23 health clinics located throughout
Montgomery County operating at regularly scheduled times in churches,
government buildings, public housing facilities, medical office
buildings and on three-fully equipped mobile medical vans. Days
and times of clinic operations vary. Clinics offer both appointment
and walk-in service. Payment for treatment is determined by a sliding
fee scale based on the patient's family income, size and ability
to pay. |
