Accept referrals from within Behavioral Health and Services for adults with mental health, substance use and co-occurring disorders and other impairments. Service plans are created and monitored by JSSA professionals on an ongoing basis. Also offer caregiver and family consultation, individualized in-home support services and hospice care.
