| Provide In-Home Aide Services/personal care service and/or chore service to eligible frail elderly and adults with disabilities to help them remain in their own homes. An eligibility determination is required and a clinical assessment is provided for eligible individuals. Program participants must agree to pay any In-Home Aide Services fee required on a sliding scale. Eligible applicants may be placed on a wait list for this program. In addition, for residents who use the Medical Assistance Personal Care Program, the In-Home Aide Service program may provide emergency back-up coverage on a temporary case-by-case basis. Provides financial reimbursement to individuals with long-term or severe physical disabilities who require attendant services such as in-home assistance with personal care, household chores, and transportation. Community Pathways covers services such as residential, CSLA, day and supported employment services. Additionally, it covers Family and Individuals Support Services for eligible individuals. New Directions allows an individual, with assistance from a Fiscal Management Service (FMS) and a person called a Support Broker, to manage their budget, hire and supervise their own staff and make decisions about how their services are provided. |

Program is designed to assist younger adults (up to age 65) who want to leave nursing facilities for homes of their own in the community. Services may include comprehensive service coordination, fiscal intermediary, attendant care, environmental accessibility and adaptations, assistive technology, consumer training, and transition services including funding for essential furniture and household supplies. Administered by The Coordinating Center. |

| Medicaid Waiver for Older Adults |

Services include personal care, respite care, home-delivered meals, environmental assessment family or consumer training, personal response system, etc. A list of individuals interested in receiving waiver services is being maintained. To place a name on the Waiver Services Registry, call 1-866-417-3480 (Toll Free). Registrants will be notified as openings become available. Waiver services are available immediately for eligible nursing home residents (wishing to leave the nursing home), whose stay has been paid by Long Term Care Medicaid for 30 days or more. |

| Rare and Expensive Case Management (REM) |

| This program is intended to provide specialized services to people with extraordinary health care needs. People who may benefit from REM are those with a diagnosis or care needs that are beyond the usual scope of Medicaid managed care companies. The Rare and Expensive Care Management Program (REM) is a Maryland Medicaid program that is part of Maryland's managed care system known as HealthChoice. It is administered by The Coordinating Center. |

| Provides personal care to people who are 65 years old or older, have a disability and are at risk of entering a nursing home, whose income is not more than 60% of the State's median income per month, and whose assets meet the eligibility requirements. | Back to top
