The Department of Veterans Affairs furnishes upon request, at no charge to the applicant, a Government headstone or marker for the unmarked grave of any deceased eligible veteran in any cemetery around the world, regardless of their date of death. For eligible veterans that died on or after Nov. 1, 1990, VA may also provide a headstone or marker for graves that are already marked with a private headstone or marker. When the grave is already marked, applicants will have the option to submit a claim for either a traditional headstone or marker, or a new medallion. When burial or memorialization is in a national, post, or state veterans' cemetery, a headstone or marker will be ordered by the cemetery officials based on inscription information provided by the next of kin. When burial is in a private cemetery, VA Form 40-1330, Application for Standard Government Headstone or Marker, must be submitted by the next of kin or a representative, such as a funeral director, cemetery official or veterans counselor, along with veterans military discharge documents, to request a Government-provided headstone or marker. Applications can be mailed or faxed.

Bronze Medallions: The new item can be furnished instead of a traditional government headstone or marker for Veterans whose death occurred on or after Nov. 1, 1990, and whose grave in a private cemetery is marked with a privately purchased headstone or marker. The medallion is available in three sizes: 5 inches, 3 inches, and 1 1/2 inches in width. Each bronze medallion features the image of a folded burial flag adorned with laurels and is inscribed with the word "Veteran" at the top and the branch of service at the bottom. Next of kin will receive the medallion, along with a kit that will allow the family or the staff of a private cemetery to affix the medallion to a headstone, grave marker, mausoleum or columbarium niche cover.
