Select “Provider vacancy/fee update form”

How to Report Changes in Your Program Information to LOCATE: Child Care

When parents call for child care referrals LOCATE: Child Care will share information about your program with them. If you have changed your fees, changed your hours, added a pet to your home, etc. it is important to report that to LOCATE: Child Care so parents are well informed.
