Adult Health Care Services - top

Cancer Prevention and Screening

Provides cancer screening and case management services to eligible low - income and uninsured county residents. Also provides care coordination and patient navigation for clients to access cancer screenings. Targeted cancers are: colorectal, oral and prostate.

Injury Prevention Program

This program works in collaboration with community partners to reduce death and disabilities from preventable injuries, especially among young children. The program provides public awareness about various injury risks for children, professional consultation and training for human service providers. Safety products such as child safety seats, bike helmets and reflective devices are distributed to individuals in need.

Montgomery Cares

Montgomery Cares is a program that provides primary health care to medically uninsured, low-income adult residents of Montgomery County. This program is funded in part by Montgomery County to help support a network of independent, nonprofit clinics. The government does not operate these clinics. Each clinic has its own history, culture, traditions and additional funding sources. Medical professionals staff all clinics.

Montgomery Rx

Montgomery Rx, a joint effort of Montgomery County and the National Association of Counties (NACo), which can save you money on prescription medications. The card is FREE and available to all Montgomery County residents, regardless of age, income or existing health insurance.

Women’s Cancer Control Program

Provides yearly breast and cervical cancer screening and follow-up for low income, uninsured/underinsured female residents of Montgomery County. Limited nursing case management services, as well as assistance linking to other community and state resources as appropriate, are available. Screening services are provided at various contracted physician and radiology facility sites throughout the County.

Children’s Health Care Services - top

Administrative Care Coordination Unit (ACCU)/Ombudsman Program

The goal of the unit is to improve the effectiveness and efficiency of the Medicaid Managed Care Program, HealthChoice. The ACCU serves as the single point of entry for outreach, education and care coordination of HealthChoice enrollees.

The Ombudsman investigates disputes between the Healthchoice enrollees and the Managed Care Organizations (MCOs). These cases are referred by the State Complaint Resolution Unit to the local ombudsman. The following actions may be taken to resolve the dispute: Educating the MCO or the enrollee, mediating disputes, assisting the enrollee in negotiating the MCO's internal grievance process and advocating on behalf of the enrollee.

Care for Kids

Care for Kids, is a Montgomery County funded program administered by the Primary Care Coalition. The Program offers a solution for needy families by providing a “medical home” for uninsured children where they can receive well and sick care, prescription and limited specialty care. Through partnerships and subcontracts with Kaiser Permanente, Community Clinic Inc., private doctors, hospitals, and other health care providers and organizations, Care for Kids promotes a continuity of care that encourages low-income families to maintain and improve their health status.

Childhood Lead Poisoning Prevention

The program provides case management for children who have blood lead levels of at least 10 micrograms per deciliter; Education and outreach to schools, child care facilities, landlords, residents, and the medical community about lead poisoning; Monitoring incidences of childhood lead poisoning and environmental lead hazards in our community; Blood lead screening; Promotion of lead-safe environments for children in Montgomery County.

Children with Special Needs Case Management

The Children's Medical Services provides care coordination for uninsured, low income children with severe, chronic or handicapping health conditions by linking them with specialists and coordinating their care. The State provides a grant to the County, which funds the Care Coordinator through the Care for Kids Program. The Care Coordinator, located at the Primary Care Coalition, works closely with Care for Kids families and primary care providers to assist them in completing the application and referrals.

Head Start Health Services

The Head Start program serves eligible 3 and 4 year-old children of low-income families, providing comprehensive educational, social, medical, dental, mental health, and nutrition services. Educational, speech/language, and health screenings are provided for each child.

Immunization Outreach and Education Program

Provides immunization outreach and educations services to County residents, private medical providers, WIC providers and clients, schools, child care providers, and other community groups. Provides case management services to children who are not up-to-date with immunizations.

Immunization Vaccination Administration

The program integrates services with staff at area health centers and school health clinics to deliver walk-in immunizations to children, from birth to age eighteen, and eligible adults, according to federal and state regulations and the "Vaccine for Children" criteria.

Infant at Risk Case Management Program

A free service for at risk mothers and infants from birth to one year residing in Montgomery County. This early intervention program ensures that identified infants and their families receive appropriate support to address their medical and psychosocial difficulties.

Maryland Children’s Health Insurance Program (MCHP)

Free or low-cost insurance coverage for children from birth to age 19 and for pregnant women offered by the State of Maryland. Comprehensive medical, dental, pharmacy, specialist and hospital care covered. Applicant must be a citizen or a legal U.S. resident. Proof of citizenship or residency and identity are required.

School Based Health and Wellness Centers

School Based Health/Wellness Centers operate in 4 elementary schools and 1 high school.  The centers provide comprehensive health, mental health and social services, case management and health promotion in familiar and student and family-friendly environments.

School Health Services

School Health Services assures and promotes the health, safety and well-being of students enrolled in public elementary, secondary and special education schools in Montgomery County Public Schools.  Basic services include: first aid/emergency care, medication and treatment administration, hearing and vision screenings, case management of students with chronic health conditions, health promotion/education, and linking students to health care providers and other resources. School Community Health Nurses (RNs) and School Health Room Aides (Certified Nursing Assistants) provide a public health presence in schools to prevent and contain communicable diseases, assure that students are appropriately immunized, and respond to school and community wide emergencies. SHS also provides specialized programs including Head Start Health Services, School Based Health/Wellness Centers, Teen Pregnancy Prevention and the School Health Services Center at the MCPS International Student Office.

Disease Control/Outbreak Investigation

Investigates case reports of residents with communicable diseases. Epidemiological investigations link cases of illness to prevent or control outbreak situations. Monitors endemic and other epidemic health situations. Communicable disease consultations provided to physicians, hospitals, nursing homes, the public or any organization or agency with questions. Provides information and consultation about control measures for many communicable diseases.

HIV Case Management

This program provides HIV Seropositive Services, including: case management, a diagnostic evaluation unit, HIV outpatient clinic services, HIV dental care and nutritional and addictions counseling. On-going support groups are available for anyone who is HIV positive or has AIDS. Home health care services are offered as needed. Education and outreach to at risk populations are also provided.

Refugee & Migrant Workers Health Assessment

STD/HIV Clinical Services

The Patient Services Diagnostic Unit provides additional lab test and diagnostic evaluations of persons tested positive for HIV. Patients diagnosed with STD are also treated.

STD/HIV Testing Program

Services include diagnostic testing, exams and treatment of STD's and epidemiological follow up. Free, anonymous and confidential HIV testing is also offered. All persons tested for HIV receive pre- and post-test counseling.

Licensure and Regulatory Services - top

The office licenses and inspects, under both county and state regulations, numerous facilities, including: assisted living facilities; nursing homes; domiciliary care homes; group homes, food service facilities; facilities offering bingo, raffles and amusements; massage establishments; swimming pools; tanning facilities; transient lodging; and video games facilities. Minority Health Initiatives/Program - top

African American Health Program (AAHP)

The program addresses health disparities that are disproportionately affecting African Americans in the areas of diabetes, HIV/AIDS, cardiovascular disorders, infant mortality, oral and dental health, and other chronic diseases. Services provided include nurse case management for pregnant women who are at risk or high risk for an adverse outcome, oral and dental health screenings, nutrition and fitness activities, diabetes education, hypertension screening and education, HIV counseling and testing and educational sessions to African American communities in Montgomery County, MD. The program also offers mini-grants to community based organizations to help outreach to communities.

Asian American Health Initiative (AAHI)

The Asian American Health Initiative (AAHI) was established to help eliminate health disparities that exist between Asian Americans and their non-Asian counterparts. The mission of the AAHI is to identify the health care needs of the Asian American Community, to develop culturally competent health care services, and to implement health education programs that are accessible and available for all Asian Americans in Montgomery County. AAHI provides education and awareness about hepatitis B, cancer (breast, cervical, colorectal, and prostate), diabetes, osteoporosis, and tobacco control through seminars, health events, and community outreach. In addition, AAHI conducts on-site osteoporosis screenings and offers breast, cervical, colorectal, oral and prostate cancer screenings to eligible Montgomery County residents. To provide culturally and linguistically appropriate services and interventions to Asian Americans, AAHI also developed a Patient Navigators Program. Through this program, trained Asian American medical interpreters may provide translation services at community clinic sites as well as assist with navigation of health care access over the telephone hotline.

Health Promoters Program “Vias de la Salud”

The mission of the program is to improve the health and wellbeing of the low income Latino community of Montgomery County, through the training and empowerment of Latino health promoters using strategies of promotion and healthy lifestyle behaviors, increasing access to care and advocacy for policies that impact and benefits the community.

Latino Asthma Management Pilot Program

This pilot program is designed to increase the knowledge of Latino parents of children with asthma regarding the condition and its management, and increase awareness and utilization of pediatric clinical services.

Program for Licensure of Foreign-Trained Health Professionals

This program provides a comprehensive, integrated and coordinated approach to effectively address the needs and decrease the challenges and barriers foreign-trained nurses encounter in Maryland in obtaining their nursing licenses. The program includes: a support and guidance system, academics, practical exposure to the U.S. healthcare system, and mentoring.

Office of Vital Records (Birth and Death Records) - top

Issues certified death certificates for 30 days from date of death, when filed at County office. Disinterment permits, (no charge), are issued with the assistance of a licensed funeral home. Issues Statement of Age cards (not State Birth Certificate) for verification of person(s) born in Montgomery County only. Birth information issued only to birth person or parents of birth person unless legal documentation presented. This office also begins the birth certificate process for a Montgomery County Home Birth.

Eligibility Requirements: Deceased must have died in the State of Maryland.  Birth person must be born in Montgomery County to receive verification. 

Payment Description:

  • $20 for certified death certificate
  • $4 for Statement of Age card.

  • Payable by check, cash or money order.
  • Payable to: Montgomery County Government.

Public Health Emergency Preparedness and Response Program - top

Montgomery County’s Public Health Emergency Preparedness and Response Program (PHEPRP) plans, prepares, practices and remains alert for events affecting public health. In the event of a public health emergency, PHEPRP and the Maryland Department of Health and Mental Hygiene are prepared to quickly follow protective public health procedures. PHEPRP’s Advanced Practice Center (APC) is one of eight centers across the country that develops emergency preparedness, response tools and resources for other local health departments. The Montgomery County APC’s focus includes providing public health emergency awareness and education, systems to dispense medicines and supplies, and emergency preparedness for vulnerable populations such as young children and seniors.

Health Promotion Program

This program provides a combination of training, consultation, coalition building, health information and program development assistance to community groups to help change attitudes and behaviors related to alcohol, tobacco and other drug use. The program administers grants to community groups for substance abuse prevention; provides nutrition planning and information related to obesity prevention; coordinates traffic safety and injury prevention programs, child abused prevention and parenting programs for high-risk parents, and a health information outreach program targeting African American churches to help reduce health disparities.

Substance Abuse Prevention Program

The program seeks to create healthy communities and families by preventing, reducing or limiting the use of alcohol and other drugs. Most programs are implemented by contractors who perform a variety of services focusing on systemic changes in policies, laws and individual awareness as well as direct services. Direct services target at risk families, some with parents enrolled in substance abuse treatment, and at risk children and adolescents. Substance Abuse Prevention programs implemented: Dare to Be You, Strengthening Families Program and Drawing the Line on Under 21 Alcohol Use.

The program provides a wide range of services, including youth prevention activities, minority education and outreach on the effects of tobacco use, enforcement, and smoking cessation.

Under One Roof Medical Clinic

The Under One Roof Medical Clinic is a joint collaboration of the Montgomery County DHHS Behavioral Health and Crisis Intervention Services and Public Health Services. The main objective of the clinic is to provide, under one roof, primary medical care to County residents enrolled in the Outpatient Addiction Services programs. This program provides medical services to adults with behavioral health and/or addiction issues.

  • Partners with Montgomery County: This program is funded through partnerships with DHHS via the Maryland Community Health Resources Commission and Montgomery Cares.

Administrative Care Coordination Unit (ACCU)/Ombudsman Program

The goal of the unit is to improve the effectiveness and efficiency of the Medicaid Managed Care Program, HealthChoice. The ACCU serves as the single point of entry for outreach, education and care coordination of HealthChoice enrollees.

The Ombudsman investigates disputes between the Healthchoice enrollees and the Managed Care Organizations (MCOs). These cases are referred by the State Complaint Resolution Unit to the local ombudsman. The following actions may be taken to resolve the dispute: Educating the MCO or the enrollee, mediating disputes, assisting the enrollee in negotiating the MCO's internal grievance process and advocating on behalf of the enrollee.

Community Health Nursing Case Management Program, Maternal & Child

These programs provide on-site and in home Community Health Nurse Case Management services to pregnant women and children up to age two. The case management services include education, assessment and referral, and assistance to clients in accessing health, economic and self-sufficiency services and programs.

Family Planning Program (Reproductive Health)

Reproductive health care/GYN consultation services for low income/uninsured teens and women of childbearing age. Services include: physical examinations; pelvic examinations; breast examinations; pap smears; testing for pregnancy; birth control methods; routine GYN lab work for sexually transmitted diseases as well as HIV counseling and testing; and basic medication for sexually transmitted diseases and other GYN conditions or infections.

Improved Pregnancy Outcomes Program (FIMR and/or CAT)

The goal of the Improved Pregnancy Outcomes Program is to improve birth outcomes and enhance the health and well-being of women, infants and families in Montgomery County by strengthening community resources and service delivery systems. The program consists of two components: the Fetal Infant Mortality Review Board (FIMR) and the Community Action Team (CAT). FIMR staff review cases of fetal and infant losses and review in-depth a select number of cases. The FIMR Review Board gathers information related to poor pregnancy outcomes, conducts case reviews to identify preventable systems-level issues and makes recommendations for change. The CAT, an advisory and advocacy group, brings together leaders who take the recommendations and implement strategies that will benefit future mothers and infants.

Maternity Partnership Program

Prenatal services through three local hospitals, Holy Cross, Washington Adventist and Shady Grove Adventist. This program provides prenatal care, including routine laboratory tests, prenatal classes and a dental screening by a dental hygienist, if referred.

Perinatal Hepatitis B Prevention Program

This program provides prevention of Hepatitis B in newborns through early identification of Hepatitis B virus infection in prenatal women.

  • Eligibility Requirements: Prenatal females, ages 12-50, who are County residents with Hepatitis B infection confirmed by lab report. Children of Hepatitis B infected mothers are also eligible for the program. Walk-in and referrals accepted. Lab report required.

Teen Parent Support Team

The program provides peer group education about rearing children, healthy relationships, and prevention of repeat teenage pregnancy. The support groups, led by school nurses and assisted by MCPS staff or Community Partners, are available in 12 - 15 high schools depending upon the annual need.

Teen Pregnancy Prevention

School nurses work with individual students and groups to promote healthy life styles and prevent risk behaviors, which can result in teen pregnancy.  Coordination among County, community partners and private partners is supported through the Interagency Coalition on Adolescent Pregancy, a consortium of community providers who work together to prevent teen pregnancy and support teen parents.  School Health nurses work with community health nurses and health care providers to access and provide case management, prenatal education and referral assistance to optimize positive birth outcomes for teen mothers and their babies.

Women’s Cancer Control Program

Provides yearly breast and cervical cancer screening and follow-up for low income, uninsured/underinsured female residents of Montgomery County. Limited nursing case management services, as well as assistance linking to other community and state resources as appropriate, are available. Screening services are provided at various contracted physician and radiology facility sites throughout the County.
