Senior Assisted Living Subsidy

How to Apply 

Please call the Senior Assisted Living  Subsidy program at 240-777-1138 to request an application.  A client who may be eligible for subsidy based on their income and assets (see “Eligibility Requirements” below) will be assessed by a Department of Health and Human Services nurse, who will visit and perform this assessment at no cost. 

Documents To Provide

  • Completed application
  • Photo ID showing name and date of birth
  • Proof of income, assets, benefits and medical expenses.  This may include bank statements, Social Security Administration or pension award letters, receipts for prescriptions and doctor's visits showing out-of-pocket costs (versus what insurance pays).

Eligibility Requirements

  • Applicants must be a Maryland resident
  • Must be 62 years or older
  • Must require 24-hour supervision as documented by a nurse from the Department of Health and Human Services.  This assessment is provided on-site at no cost to the client. 
 

Income and Assets (as of July 14, 2023*):

  • For an individual:  no more than $4,350 income and $20,064 in assets
  • For a couple:  income is looked at individually, and no more than $20,064 in assets
*Income, assets, subsidy and cost of care limits can change periodically

Fees and Payments 

The Senior Assisted Living Subsidy Program limits the total amount assisted living facilities may charge a subsidy participant for their care.  The total monthly rate may not exceed $3,000 per month for the care rate.  

The maximum subsidy an applicant can currently receive is $1,056.00 per month.  In some cases, an applicant may qualify for a partial subsidy (less than $1,056.00 per month) based on their calculated monthly payment.  In most cases, the subsidy does not cover the full cost of care.  The applicant is responsible for the amount beyond the subsidy payment up to the monthly rate.  Family, friends, or others may assist in paying these costs, or negotiate the price with the facility directly.  The assisted living facility has the right to not accept residents who cannot pay the cost of care.  The relationship between the applicant and the facility is private.  The subsidy program merely helps make care more affordable.
 

FAQ's 

1.  How can I find an assisted living facility?
The applicant or their representatives are responsible for finding an assisted living facility that participates or that would be willing to participate in this subsidy program.  A list of participating facilities may be requested by calling the Subsidy Program at 240-777-1138.  Specific questions about individual facilities can be addressed to the Long Term Care Ombudsman program (240-777-3369).

2.  Is there a waiting list for the Subsidy Program?
When an application is received, the Senior Assisted Living Subsidy Program date stamps the application and places the applicant’s name on the waiting list.  Applications are reviewed and processed according to funding availability.  By regulation, the applications are served on a first come, first served basis when all steps of the application process are complete.  Due to budget limits, eligible applicants may need to wait for a space in the program.
