The Senior Care system provides case management and support services for people aged 65 years or older who are at risk of nursing home placement. Gap-filling funds to meet other needs may also be available to those who qualify. Those funds, depending on funding availability, can pay for Adult Day Care, medical supplies, Personal Emergency Response Systems (PERS), medical reimbursement, Transportation, and/or programs to support Health and Wellness.

Documentation will be requested during an in-home social work assessment during which eligibility will be assessed. Documentation may include, but is not limited to financial, medical, and identifying documentation.
An individual must be at least 65 years of age, a US citizen or qualified alien, at risk of institutionalization and must meet financial eligibility guidelines.

1. Can I receive Senior Care if I reside in a Group Home or Assisted Living?
Yes. Residents of group homes or assisted living facilities are eligible for services which are not provided by the group home or assisted living facility.

The Senior Care Program is not an entitlement program. It is grant funded through the Maryland Department on Aging. Eligibility period, approval, and waitlist time varies according to funding availability.
