Eligibility Requirements

County residents who are ages 18 and older who have a functional impairment from either a physical or cognitive disability. 

Asset eligibility is under $19,999 for a single adults and $22,999 for a couple. 

Social Services to Adults in a voluntary program and the participant must be agreeable to accepting the service and providing requested documentation for case management and eligibility review.

Fees and Payments

Financially eligible participants will not have a fee or payment for assessment/continuing case management.

FAQ's

1.  What services are provided by SSTA?

The program provides short term or continuing case management to help clients reach their maximum level of self-sufficiency.  Case management is often geared toward services such as home care, Senior Care, adult foster care and crisis intervention. Internal program referrals have their own eligibility criteria, which will be assessed for as part of the initial evaluation for services and during semi-annual reconsiderations.
