Bridges to Behavioral Health Services Transitional Psychiatry Program

Located within ACCESS to Behavioral Health Services, Bridges to Behavioral Health Transitional Psychiatry Program is designed to bridge the gap between acute psychiatric care and long-term outpatient treatment. Our program provides short-term, intensive psychiatric support for individuals who are stepping down from inpatient hospitalization, crisis stabilization units, as well as for those who are unable to readily connect with a community provider for behavioral health care needs. The purpose of the Bridges to Behavioral Health Transitional Psychiatry Program is to reduce the risk of relapse and re-hospitalization; by providing structured psychiatric care, medication management, and therapeutic support, we aim to stabilize symptoms and help individuals develop coping strategies necessary for long-term wellness.

Who We Serve

Individuals without private/commercial medical insurance. Telephone consultation and information and referral services are for all residents and professionals or referring agencies.

How to Apply

Referrals to the Bridges to Behavioral Health Transitional Psychiatry Program can be made by hospital staff, primary care providers, community mental health agencies, or by self-referral. Our team will conduct an intake assessment to determine eligibility and develop a personalized treatment plan.

Identification, proof of residency, proof of insurance and/or income if applicable.

Eligibility Requirements

For assessment and referral services, individuals must be Montgomery County residents, age 18 or older.  Individuals must be uninsured with no or low income or have Medicaid or Medicare insurance. Telephone consultation and information and referral services are available for all residents and professionals.

Fees and Payments

Fees are charged for assessments, counseling, and visits with the Bridges Transitional Psychiatry prescribers.  The program accepts Maryland Medicaid and Medicare. Clients will be responsible for co-payments.  Bridges to Behavioral Health Services utilizes the Maryland Department of Health sliding fee schedule to determine the fees for uninsured clients. To be eligible for a reduction of fees, clients are expected to provide verification of income in the form of pay stubs, bank statements, or tax returns.  Letters of support from family members or shelters are accepted.  Clients will not be denied services based on their ability to pay or their immigration status.
