Documents To Bring
Photo identification, proof of residence in the County (government issued ID or mail at current address or lease/affidavit from landlord). If client is referred by an agency requiring treatment, the referral documents should be provided.
Eligibility Requirements
Montgomery County residents ages 18 and older who are in need of urine screening to support recovery from substance abuse.
