Sliding Fee Discount is determined for patients based on:

Financial assistance is available after the sliding fee discount is applied. Montgomery County DHHS Dental Program offers two options to assist families who would like financial assistance in the following payment options: 

Payment Plan:

• Payment plans are available upon request. No additional application is needed.
• Interest-free payments can be scheduled for up to 12 months.
• Patients must complete a “Payment Plan Agreement” form and submit to the dental clinic for processing.
• Additional balances associated with new treatment can be added to an existing payment plan.

and/or

• A fee reduction/waiver is considered upon request.
• Eligibility for the dental hardship program fee reduction or waiver is determined on a case-by-case basis.
• Patients may apply for the dental hardship program fee reduction/waiver by providing copies of one of the following debt obligations for supporting documentation:
  • Child support
  • Alimony
  • Garnishments
  • Childcare expenses
  • Student, personal, asset, 401k loans etc.
  • Mortgage
  • Rental Property
  • Co-signed obligations
  • Business debts
  • Collection accounts
  • Or any other financial obligation that is affecting your ability to pay for your dental services
• Once a fee reduction/waiver request is approved, the applicable fee reduction will be applied for the patient’s account.
• Dental hardship may be determined every 6 months and considers past dental expenses; it is not an ongoing eligibility category.
• All dental expenses incurred with a 6-month time frame from the date of dental hardship determination are eligible for full or partial waiver.
• Patients can be reassessed for dental hardship eligibility every 6 months.
