Birth and Death Certificates (Vital Records)

The Montgomery County Department of Heath and Human Services issues certified birth and death certificates
Monday – Friday (except when County offices are closed).

Services are provided on a walk-in, first come/first served basis during the hours of operation.

Applicants must check-in at the security desk before proceeding to the Vital Records office.

The Vital Records office issues certified birth certificates for anyone born in the State of Maryland after 1939.
Certified death certificates are issued for individuals who died in Maryland AFTER January 1, 2015 (for individuals who died before January 1, 2015, certificates must ordered from the  Maryland Vital Records Administration).

Permits for exhumations (discontentment) are issued with the assistance of a licensed funeral home. Birth certificates are available for any individual born in the State of Maryland. To obtain a birth certificate at the Dennis Avenue Health Center in 1939 or after. Those born before 1939 should contact the Maryland Department of Heath and Mental Hygiene's Division of Vital Records at 410-764-3063.
