Course Completion Cards

You can submit this form using any internet ready device.

It will work equally well from a smart phone as it will from a desktop PC.

Please note that the AHA has changed the course completion cards. The following course completion cards are now available. The card request form (or click on the Wufoo logo below) should be submitted for all card requests. Please note that payment must be received prior to cards being forwarded to you. Currently the cost for the crads is $3.00 per card but the cost can increase ithout any prior warning. Cards are packaged as three (3) cards per page, so when ordering it would help if you request cards in that format whenever possible.

AHA Authorized Vendors

The AHA has three (3) authorized vendors. All vendors sell the AHA products for the same base price, but some may offer discounts such as free shipping.

 

 

How to Register on the AHA Network

If you are not registered on the AHA Instructor Network, youneed to create an account. Log on to https://myportal.americanheart.org/eccportal/ecc/ecc

How to request a DUPLICATE card

 

How to Replace a lost card

Duplicate cards are issued by the course instructor that taught the individuals course. The cost for the card is $5.00 per duplicate card.

The process is:

  1. the student requests the duplicate card from the Instructor of Record (IoR) .
  2. The IoR requests the duplicate card from the Training Center Coordinator (TCC) via an email request.
  3. The individual pays the $5.00 fee, who forwards the $5.00 and the following information to the TCC.

  4. The duplicate card will be created and issued by the TCC, and mailed via US Mail or inter departmental mail as appropriate to the IoR

The IoR must provide the following information to the TCC:

  • 1. Individuals Name
  • 2. Date of Course and expiration date on initial card
  • 3. Type of card to be reissued (HCP, HS etc)
  • 4. Instructor's name and AHA ID #
  • 5. Address to send the AHA Duplicate Card

No duplicate card will be issued without payment and all of the information above.
