Core Measures Set

The Core Measures are data indicators that represent Healthy Montgomery’s six priority areas. Some measures represent multiple priority areas (Cross-Cutting) and highlight the impact (Context) that social determinants of health provide to our community health improvement process. The measures can be monitored over time and compared to peer counties or to state and national benchmarks (e.g. Healthy People 2030, Maryland State Health Improvement Process).
