Unite 2 Integrate Diabetes Coalition

The UNITE 2 INTEGRATE DIABETES COALITION (U2IDC) is a key strategic activity of the Montgomery County Department of Health and Human Services and Public Health Services' Type 2 Diabetes Strategy. U2IDC will provide a forum for Montgomery County public health professionals, health care providers, social service providers, and other professionals to network and learn from each other about strategies to improve chronic disease outcomes in Montgomery County. Through a collaborative effort, resources are provided on planning, implementation, and evaluation programming in Montgomery County. The Coalition focuses on assessing needs, raising awareness of the burden of chronic diseases, and promoting uptake of prevention, education, and promotion efforts. The U2IDC’S initial priority will be to provide a forum to advance the priorities of  Maryland’s Diabetes Action Plan in Montgomery County.

GOAL: Support high-risk minority Montgomery County residents with managing and controlling Type 2 Diabetes (T2D) through early detection, addressing the social determinants of health, and promoting optimal self-care and rehabilitation.

OBJECTIVE: The U2IDC will build a comprehensive, accessible, and reliable source for Diabetes prevention and management knowledge and information sharing. The coalition will provide a platform for coalition members to share latest Diabetes research findings and evidence of best practices. In addition to educate and inform the public legislators, policymakers and key decision makers about legislation, public policies, and initiatives that relate to Diabetes prevention and self-management.

MEMBERSHIP: County government agencies, county hospital systems, minority health programs/initiatives, advocacy groups, academic institutions, community-based service providers and other stakeholders whose mission is to support minority Montgomery County residents with achieving optimal diabetes health and well-being.

MEETINGS: Quarterly coalition meetings to discuss accomplishments, strategies, and goals. Meetings will be conducted by Zoom or In-person. Intermediate, as needed conference calls or meetings to discuss key points and/or progress.
