Instruction for the form submission
- Event title: Include organization name, type of distribution, geographic location, and pre-registration or appointment required, ex: “ OFSR Drive Thru Food Distribution in Glenmont (By Appointment Only)”
- Location, start date, and time: Location name/description & street address (OFSR Church Parking Lot, 1234 OFSR Lane).
- Duration: Select how long the food distribution event will last or specify when the event will end.
- Repeat: Select the frequency of the event from the dropdown.
- Category: Select 'Food Distribution'
- Access: Select all that apply. Is any documentation required? If so, please specify here what needs to be provided (a valid photo ID, documentation confirming your address, etc.)
- Appointments Needed: If you selected "By Appointment Only" specify available appointment windows, restrictions, and required information. Such as, “Available Tues & Thurs, 10 AM – 2 PM”) Indicate if you can or can not accept same-day or next-day appointments. How to book (phone, online link, email, etc.) .
- Documentation Required: If you selected "Documentation Required" specify what residents need to bring (a valid photo ID, documentation confirming your address, etc.)
- Food Assistance Type: Select all that apply
- Age: What age group is the food distribution event for? * Select all that apply *
- Area: Select what area(s) the food distribution event is in? * Select all that apply *
- Contact name, phone, and email: Input contact information for the event’s primary organizer
- Cost: Only post free food distribution events.
- Also Show On: Click "Food" from the drop-down list.
- Weblink: Link to the event or organization webpage, social media page, or registration page if applicable.
- Description: Specify food provided: Ex. shelf-stable items, fresh vegetables, and protein.
- Additional Guidance for Clients in description box: Ex: Clients should bring their own bags, clients should park in X lot, etc.
