Program Summary

Eligibility:

LiveWell programs are free and open to insurance-eligible Montgomery County Government (MCG) and participating agency employees.
You do not have to be enrolled in a health plan, just eligible to enroll.

Program Highlights:

Earn $350 per year for making healthy choices.
Receive a free activity tracker or use your own (if compatible with Personify Health).
Sync capability with partner devices such as the FitBit, Apple Watcher, Polar Heart Rate Monitor, and many more.
Enjoy a fun, easy, and secure personalized member portal to manage all of your health and fitness information in one place.
Participate in up to four countywide step challenges per year, and create your own personal challenges.
Invite up to 10 family members to join Personify Health; although they will not receive the free fitness tracker or $350, they can access other great resources.

Registration

New employees: You will receive an email from Personify Health with registration details within three to four weeks of your hire date. If you do not receive the registration email within four weeks of your hire date, contact Livewell at [email protected].
Current employees: If you are enrolling in Personify Health for the first time, contact Livewell at [email protected] to request registration details.

Personify Health FAQs

I lost or damaged the activity tracker I received from Personify Health; what should I do?
- Each employee can request one free activity tracker when they join Personify Health. If you lose your device, you can:
  a) Purchase a new activity tracker through Personify Health (see Member Resources above).
  b) Sync a compatible activity-tracking device with Personify Health by logging into your account (click More, then Devices & Apps).
  c) Use your smartphone as a step tracker. Download the Personify Health app to your Android or iPhone. For help, contact Personify Health (see Member Resources section above).
Who can participate in the Personify Health program?
- LiveWell programs, including Personify Health, are open to insurance-eligible, Montgomery County Government and participating agency employees. You do not have to be enrolled in a health plan, just eligible to enroll. Participants can invite up to 10 family members to join the Personify Health program. Although they will not receive the free activity tracker or $350, they can access other great resources. To invite family members, log on to your Personify Health account through your device. Who can access my personal and health-related information?
- Montgomery County Government (MCG ) does not have access to your personal contact information or health information. MCG only receives aggregate data from Personify Health relating to enrollment, utilization, and the state of employee wellbeing, as a whole, within the County.
How can I earn points?
- There are so many ways you can earn points. Log on to your Personify Health account through your device to see all your options.
What types of rewards can I get?
- You can choose to have your reward dollars deposited directly into your bank account, or select a reward from the long list of options available through your Personify Health account on your device . Check it out today!
