Making a Request

To request one or more reports from MCFRS records please use our Records Request portal. This provides you with the quickest service.

Make a Request

If you prefer not to use the online portal you may also make record requests by mail or email. Please include the following information dependent on what type of report you need:

EMS Reports

To request information please provide:

  • Date of Incident
  • Approximate Time of Incident
  • Location of Incident
  • Patient's Full Name
  • Signed Medical Release Authorization (not applicable if requestor is the patient)
  • Power of Attorney (if patient is living but cannot request for themselves)
  • Letter of Administration or Executor documentation (if patient is deceased)
  • Self-addressed, stamped envelope (if report is to be mailed)

Fire Incident Reports

To request information please provide:

  • Date of Incident
  • Approximate Time of Incident
  • Location of Incident
  • Name of the property owner(s) (if known)
  • Requestor’s signature on a document requesting the report
  • Self-addressed, stamped envelope (if report is to be mailed)

Our goal is to respond within thirty (30) days receipt of all requests that comply with the requirements indicated above.
