Camp Forms
General camp information will be emailed to parents prior to the start of camp. Montgomery County Recreation uses the ePACT Network to securely collect and manage critical participant information, ensuring we can best support your child during program hours.
Families will receive a request from ePACT to complete the required electronic forms for each participating child. These forms include the Camper Health and Information Form and Camper Code of Conduct.
If applicable, families must also complete the State of Maryland Immunization Certification, Authorization for Medication, Asthma Action Plan and/or the State of Maryland Diabetes Medical Management Plan by downloading the forms below and submitting completed forms through ePACT.
All required forms must be completed prior to the first day of camp. Campers will not be permitted to participate until all required forms are submitted and approved.
If preferred, paper forms will be available for download in January 2026.
Authorization for Medication
If your child requires non-asthma medication during camp, including emergency medications such as an EpiPen, a parent/guardian must complete this form and upload it to the Shared Files section in ePACT at least 15 days before the session starts. Asthma Action Plan
If your child has asthma, a parent/guardian must complete the Asthma Action Plan and upload it to the Shared Files section in ePACT at least 15 days before the session starts. Upload issues? Email the completed form to [email protected].
State of Maryland Diabetes Medical Management Plan
If your child has diabetes, a parent/guardian must complete the Diabetes Medical Management Plan and upload it to the Shared Files section in ePACT at least 15 days before the session starts. Upload issues? Email the completed form to [email protected].
Seizure Action Plan
If your child has a seizure disorder, a parent/guardian must complete the Seizure Action Plan and upload it to the Shared Files section in ePACT at least 15 days before the session starts. Upload issues? Email the completed form to [email protected].
Out of Country Immunization Certificate
If your child resides outside the United States, a parent/guardian must complete the Out of Country Immunization Certification and upload it to the Shared Files section in ePACT at least 15 days before the session starts. Upload issues? Email the completed form to [email protected].
Camper Health and Information Form
